We are a full-service psychological services and counseling business with locations in Heber City, Kamas, and Park City, Utah that offers clinically comprehensive services of the highest quality. Each counseling session or evaluation is an opportunity for us to help an individual, child or family by finding strengths, any limitations, and unique needs, and providing helpful and practical recommendations for improvement and success. Recognizing that best practices are those that account for the uniqueness of each person, thus our experience with you or your child is truly customized.

The following is a description of the available psychological and counseling services. The price listed by each service below is the average amount a client can expect to pay for the service. Expansive Horizons charges a flat rate of $100 per hour, so actual charges may vary based on complexity of each case. Districts, agencies, and/or individuals have many options when securing services depending on individual needs.

Complete Psychological Initial Evaluation and comprehensive report:
•Review of student records
•Meeting with referral team
•Cognitive (IQ) assessment
•Academic assessment including Curriculum Based Measurements in Reading, Writing, and Math
•Social/Emotional assessment including functional behavioral assessment and behavior intervention plan
•Observation of student
•Interview with teacher, parent, student
•Consultation with other professionals
•Gathering of data
•Report writing (interpretation of assessment results, accommodations and interventions)
•Multi Disciplinarian Conference (MDC) and IEP combination meeting

Complete Psychological Re-Evaluation: (Dependent on necessary assessments)
•Review of records to determine what assessments are needed
•Perform necessary assessments
•Report writing
•Multi Disciplinarian Conference (MDC) and IEP combination meeting

Dyslexia Evaluation:
•Portions of Woodcock Johnson III Tests of Cognitive Abilities (portions related to auditory processing)
•Portions of Woodcock Johnson III Tests of Achievement (portions related to reading, writing, listening comprehension)
•Grays Oral Reading Test Fourth Edition (GORT 4)
•Comprehensive Test of Phonological Processing (CTOPP)
•Additional tests in order to form unitary and interpretable domains as per the Cross-Battery approach

A Dyslexia Evaluation report will be composed, including assessment scores, analysis, and educational implications. Finally, an interpretive conference to discuss the Dyslexia Evaluation will conclude the evaluation process.

Autism/Pervasive Developmental Disorder Comprehensive Evaluation:
•Review of student records
•Meeting with referral team
•Cognitive assessment
•Academic assessment
•Social/Emotional assessment including functional behavioral assessment and behavior intervention plan- BASC
•Autism Battery- SCQ, ADI-R, ADOS
•Observation of student in home and at school
•Interview with teachers, parents, student
•Consultation with other professionals
•Gathering of data
•Report writing (interpretation of assessment results, accommodations and interventions)
•Multi Disciplinarian Conference (MDC) and IEP combination meeting

Attention Deficit Disorder/Attention Deficit Hyperactivity Disorder Comprehensive Evaluation:
•Review of student records
•Meeting with referral team
•Cognitive assessment
•Academic assessment
•Social/Emotional assessment including functional behavioral assessment and behavior intervention plan
•ADD/ADHD scales - Conners, IVA Plus or Wisconsin Card Sorting Test, and Children's Color Trails Test
•Observation of student
•Interview with teacher, parent, student
•Consultation with other professionals
•Gathering of data
•Report writing (interpretation of assessment results, accommodations and interventions)
•Multi Disciplinarian Conference (MDC) and IEP combination meeting

Single assessment - cost per assessment, see below

Cognitive assessment (IQ) - utilizing the Wechsler Intelligence Scales. Includes administration of tests, written report including scores and analysis, and interpretive conference to discuss results.

Academic assessment - assessing reading, writing, math, oral language, phonological processing, etc. utilizing the Woodcock Johnson III Tests of Achievement or Wechsler Individual Achievement Tests. Includes administration of tests, written report including scores and analysis, and interpretive conference to discuss results.

Reading assessment - measuring rate, accuracy, and comprehension utilizing the Grays Oral Reading Test Fourth Edition (GORT 4). Includes administration of tests, written report including scores and analysis, and interpretive conference to discuss results.

Phonological processing assessment - measuring phonological awareness, phonological memory, and rapid naming utilizing the Comprehensive Test of Phonological Processing-Second Edition (CTOPP). Includes administration of tests, written report including scores and analysis, and interpretive conference to discuss results.

Additional Support Services: Billed at an hourly rate of $100 per hour
•Development of balanced educational programs, including identification and implementation of a continuum of services for special needs students
•Response to intervention services
•Providing counseling services to individuals, groups, and classrooms
•Crisis intervention services
•Providing play therapy services
•Developing effective inclusion strategies, including modification and adaptation of curriculum
•Designing functional behavioral assessments and behavioral modifications
•Assist in designing a special education referral process, utilizing Teacher Assisted Team and Child Study Team
•Staff development to support all related education processes and programs, and student and staff needs
•To meet specific needs, individualized consulting and support services are available

Further support with the Psychoeducational Evaluation, i.e. consultation with school personnel, teachers, or other relevant service providers will be billed at the rate of $100 per hour. Travel and lodging is in addition to the hourly fee and payable by the client.

Many options exist in providing services to your agency including:
•A daily rate fee
•Per evaluation as described above
•Per hour
•Combination of daily rate and per evaluation
•Short-term contracts
•Annual contracts

Assessment protocols are in addition to the evaluation unless provided by the agency or district. For example, Wechsler Intelligence Scales for Children - 4th edition protocol - $25
